Abiqua Academy COVID-19 Testing Consent

Abiqua Academy will be able to offer COVID-19 tests to students to self administer. A consent form must be turned in prior to testing. Please fill out the form below that has been provided by the Oregon Health Authority. The form can be emailed to Sheri.Pilgrim@AbiquaAcademy.org or physically turned in to the front office. Abiqua Academy is only permitted to test students with this form, no exceptions will be made. It is still advised to get a COVID-19 test by your health provider.

The Oregon Department of Education (ODE), in collaboration with Oregon Health Authority (OHA), has published Ready Schools, Safe Learners guidance to outline health and safety requirements to reduce the risk of COVID-19 transmission in Oregon’s K-12 schools. To further mitigate the risk of COVID-19 transmission in Oregon’s K-12 schools, OHA will support COVID-19 testing in public and private K-12 schools in Oregon offering partial or full in-person instruction for students and staff who display symptoms of COVID-19 or who have been exposed to COVID-19 in the school setting. The Federal Government has allocated Abbott BinaxNOW rapid point-of-care antigen tests to states to support COVID-19 testing. These tests are available to support testing in K-12 schools during the 2020-2021 school year.
